What we ask from you:
- We ask you to submit a signed Registration and Permission to Use form. This will allow Booster Stores to use your logo and graphics on your online store for the purpose of sales to your store's visitors.
- You need to provide at least one logo in .psd format. If you can not provide a logo in .psd format, we can accept other formats subject to conversion, or we can assist you in designing a logo, at cost, but with no upfront or out-of-pocket expense to you.
- We need the name and address of the high school/team/club so we can issue commission checks.
- If you are a not-for-profit organization, we need your Tax ID#. Otherwise we need a social security #.
Commission checks are issued within 45 - 60 days after month's end. Since we have a 30 day money back guarantee, we need to wait to make sure all returns have been handled before commission checks are issued. Checks will only be issued if they exceed $100; otherwise the commission will be rolled over to the next month.
